Please mail the white form to PO Box 361, Marcy, NY 13403 (keep the yellow form for your records). Paper estimates cannot be faxed or emailed.
Once we receive your properly signed agreement we can set up your account and will schedule your first treatment to be done as soon as your Technician is able to fit it in.
